    SOME ADVANCED CARDIAC PROCEDURES

  • rightTick Intravascular Ultrasound (IVUS)

  • Intravascular ultrasound (IVUS) allows medical professionals to internally examine blood vessels. It uses sound waves to detect any obstructions that may compromise blood circulation, and is also used to evaluate soft tissue. IVUS is a less invasive technique that uses small devices and advanced technology for assessments that are gentler on your body.

  • These small devices can take live images from the inside of the blood vessels and are typically used to examine the vessels close to the heart (coronary arteries). It can also examine other blood vessels.

  • A small tube (catheter) is used to reach and assess the tissues of blood vessels. A healthcare professional guides the catheter through a cut, usually located in the groin, to access the evaluation site.

  • The tip of the catheter consists of an ultrasound probe that generates high-frequency sound waves, which reflect off the walls of blood vessels, producing echoes. A computer transforms these sound waves into images in real time.

  • Benefits of IVUS:

    • It detects the presence of narrowed or blocked blood vessels.

    • It evaluates the degree of plaque accumulation within arteries (atherosclerosis) and other possible obstructions.

    • It assesses if further interventions, like coronary artery bypass surgery, are necessary.

    • It helps design and implement treatments that use a hollow mesh apparatus (stent) to keep the blood vessels open.

  • rightTick Rotablation

  • Rotablation angioplasty is a technique that uses a high-velocity, diamond-coated burr at the catheter's end to grind down tough, calcified plaque from arteries, especially in the coronary arteries. This method is applied when plaque is too calcified for conventional balloon angioplasty and seeks to re-establish blood circulation to the heart by conditioning the artery for a stent placement. It is a procedure with minimal invasiveness, generally conducted in a Cath Lab.

  • Procedure:

  • Local anaesthetic is given to the patient, then a catheter is placed into an artery, typically in the groin or arm, and it is then directed towards the obstruction using X-ray imaging. The unique, spinning burr is inserted via the catheter to break down the plaque, followed by balloon angioplasty and stent placement, which is usually done to fully expand and open the artery.

  • Patients typically remain in the hospital for one to two days for observation, and it is advised to take several days off before returning to regular activities. Intense activities should be postponed for roughly a month.

  • The recovery typically occurs faster than open heart surgery, and the prognosis is usually favourable; however, patients might need to use blood-thinning medication indefinitely and regularly follow up with their doctors.

  • rightTick TAVI & TAVR

  • TAVI (Transcatheter Aortic Valve Implantation) and TAVR (Transcatheter Aortic Valve Replacement) are minimally invasive techniques used to replace a constricted/narrowed aortic valve. Treatment avoids open heart surgery as the new valve is implanted through a catheter.

  • The aortic valve is responsible for regulating blood flow from the heart to the rest of the body. If heart valves become rigid over time, there is a high chance that the patient has aortic valve stenosis. In that case, the heart works excessively to pump blood through this narrow opening of the valve to the rest of your body. This increases the chances of heart failure.

  • Procedure:

  • A thin tube called a catheter is inserted, often through an artery in the leg. The catheter transports the new, compressed valve to the heart. Once in place, the new valve is expanded by a balloon or the valve itself. After this, the new valve takes over the function of the old one and makes it easier for the blood to flow unobstructed throughout the body.

  • Benefits of TAVI and TAVR:

    • It is minimally invasive and does not require open heart surgery

    • The patient has a shorter hospital stay and quicker recovery compared to conventional surgery

    • Symptoms such as shortness of breath, chest discomfort, and weariness can be relieved easily

    • It is often recommended and beneficial for individuals for whom open-heart surgery is too risky, owing to age or other health factors

  • rightTick Minimally Invasive Cardiac Surgery (MICS)

  • MICS or Minimally Invasive Cardiac Surgery uses small incisions on the chest rather than the large cuts in traditional open heart surgery. It avoids cutting the breastbone and, as a result, there is less discomfort, fewer scars, a lower risk of infection, and quicker recovery. Common procedures include coronary artery bypass grafting (CABG), valve repair or replacement, and atrial septal defect closure.

  • Benefits of MICS:

    • Smaller incisions lead to reduced scarring

    • Patients feel less pain and require fewer harsh medicines.

    • Patients can return to routine activities sooner

    • Reduced risk of infection or other complications than to conventional open-heart surgery

    • MICS usually causes less blood loss during the operation

  • What conditions does it treat ?

  • Who is this for ?

    • Not all patients qualify for MICS
